Common use
Alendronate is a bisphosphonate medication used to treat and prevent osteoporosis in postmenopausal women and men, as well as bone loss caused by steroid use. It helps increase bone mass and reduce the risk of fractures, especially in the spine and hips. Alendronate works by slowing down the breakdown of bone, allowing the body to rebuild stronger bone tissue. It is commonly prescribed for long-term bone health maintenance.
Dosage and direction
Alendronate is usually taken once daily or once weekly by mouth, depending on the condition and dosing form. It must be taken first thing in the morning with a full glass of water, at least 30 minutes before any food, drink, or other medications. After taking the tablet, you must remain upright (sitting or standing) for at least 30 minutes. Following these instructions is essential to prevent irritation of the esophagus.
Precautions
Before taking alendronate, tell your doctor if you have problems with your esophagus, low calcium levels, kidney issues, or difficulty sitting upright. It is not suitable for people who cannot remain upright after dosing or those with certain digestive disorders. Ensure you’re getting enough calcium and vitamin D while on this medication. Regular dental check-ups are advised, as rare jaw bone problems have occurred.
Contraindications
Alendronate is contraindicated in individuals with abnormalities of the esophagus that delay emptying, and those who cannot sit or stand upright for at least 30 minutes. It should not be used in people with low blood calcium (hypocalcemia) until the condition is corrected. Patients with severe kidney impairment may not be suitable candidates. Known allergy to alendronate or any of its components also precludes use.
Possible side effect
Common side effects include stomach pain, acid reflux, constipation, diarrhea, and muscle or joint pain. Some users may experience esophageal irritation, ulcers, or difficulty swallowing, especially if the dosing instructions are not followed. Rarely, it can cause jaw bone problems (osteonecrosis) or unusual thigh bone fractures. Contact your doctor if you develop chest pain, severe bone pain, or jaw discomfort.
Drug interaction
Alendronate should not be taken at the same time as calcium supplements, antacids, or other oral medications, as these can reduce its absorption. Wait at least 30 minutes after taking alendronate before consuming any other substances. Some medications for stomach acid, such as PPIs, may also affect absorption or increase side effects. Always inform your doctor of all medications and supplements you take.
Missed dose
If you miss a daily dose, skip it and resume your regular schedule the next day—do not double up. If you miss a weekly dose, take it the next morning after you remember. Then return to your original schedule. Do not take two doses on the same day.
Overdose
Overdose of alendronate may result in low blood calcium, low blood phosphate, or serious digestive tract irritation. Symptoms can include muscle cramps, numbness, or severe stomach discomfort. Seek medical attention immediately if an overdose is suspected. Do not induce vomiting or lie down after taking an extra dose.
Storage
Store alendronate at room temperature, away from heat and moisture. Keep the tablets in their original container and out of the reach of children and pets. Do not use expired medication. Dispose of unused medicine as instructed by your pharmacist.
